Patient Messaging & Devices

Why am I getting these SMS messages?+
Your doctor enrolled you with a medical device to record readings regularly. Intelligent Health Partners (iHP) securely shares those readings with your doctor. A minimum of 16 readings each month is typically needed for effective monitoring.
Who is iHP?+
Intelligent Health Partners (iHP) supports remote patient monitoring and care management by sharing your device readings with your provider and enabling timely follow‑up.
Whom do I contact for issues with the device?+
Please contact your doctor's office, or reply to the message you received and our team will assist you.
I'm taking readings but still getting SMS reminders+
We apologize—your readings may not be reaching our portal due to a connectivity issue (e.g., network or Bluetooth on some devices). We will reach out to help resolve the problem.
How do I opt out / opt in to messages?+
Reply STOP to opt out of future messaging. Reply START to opt back in.
How do I see my device reading history?+
Visit our patient portal atpatients.intelligenthealthpartners.com. You can use your mobile phone and/or email to create or reset your password. If you have any issues, please message us.

Programs & Eligibility

Which patients are eligible for CCM?+
Patients with two or more chronic conditions expected to last at least 12 months, presenting significant risk of death, acute exacerbation/decompensation, or functional decline, are generally eligible.
Who qualifies for RPM?+
Patients who benefit from regular physiologic data monitoring (e.g., BP, weight, glucose, pulse oximetry) and related care management can be enrolled when medically necessary and ordered by a provider.
Can CCM and RPM be billed for the same patient?+
Yes, if criteria for both programs are met and time is not double-counted. We help configure workflows and documentation to remain compliant.

Reimbursement & Compliance

What codes are used for CCM?+
Typical CCM codes include 99490, 99439 (add-on), 99487/99489 (complex), and 99491/99437 (physician time). Coverage varies by payer; verify current policies for your locality.
What codes are used for RPM?+
Common RPM codes: 99453 (setup/education), 99454 (device supply), 99457 (first 20 minutes), 99458 (each additional 20 minutes), and 99091 (data review per 30 days).
How do you ensure compliance?+
We provide standardized workflows, time tracking, care plan documentation, and audit-ready records aligned with payer and CMS guidance. We also train staff and monitor quality.

How long does implementation take?+
